What does programme oversight need to see?

Effective oversight needs four things reported honestly: whether scope has changed, whether the benefits case still holds, whether the critical path has moved, and which risks have become issues. Reporting that shows only status colour and spend to date tells a board almost nothing about whether the programme will deliver.

Why do transformation programmes usually fail?

They usually fail on adoption rather than delivery: the system goes live, the process changes on paper, and the organisation continues working the old way because roles, incentives and reporting were never changed. Benefits are claimed at go-live and never measured afterwards, so the shortfall is invisible until the next planning round.
