How do you choose between modernising a mainframe and replacing it?

Modernisation keeps the existing application and moves it: rehosting to different infrastructure, re-platforming onto a supported runtime, or refactoring the code itself. Replacement swaps the application for a package or cloud service. The decision turns on how much business logic is locked in the legacy code, what it costs to run, and whether the data model can move cleanly.

What is the difference between a data warehouse migration and a platform migration?

A data warehouse migration moves stored data and the reporting built on it, so the risk sits in reconciliation: the same query must return the same number on both sides. A platform migration moves the running application and its integrations, so the risk sits in behaviour and cutover. They are usually sequenced separately for that reason.
